1. Fill out and submit our volunteer application below
2. Email us the following clearances to carecenterfoundation@aol.com. You can obtain these clearances by clicking the links below and following the instructions.
   1. PA Child Abuse History Clearance
   2. PA State Police Criminal Record Check
   3. Federal Bureau of Investigation (FBI) Criminal Background Check
